Clinical History:
The patient was a 15 year old male with 3 episodes of hemoptysis over
several weeks. Review of symptoms was remarkable for a 30 pound
weight loss over 5 months which the patient attributed to dieting.
Clinical Physical Exam:
Unremarkable
Clinical Labs:
Non-contributory
Clinical Differential Diagnosis:
Lymphoma
Imaging Findings:
A chest film showed a large lesion in the right upper lobe. Chest CT
exam showed that lesion, and three smaller ones.
Imaging Differential Diagnosis:
Metastatic disease
Operative Findings:
A left orchiectomy was performed. A non-palpable tumor was found.
Pathological Findings:
Pathological examination of the surgical specimen revealed findings
characteristic for an embryonal cell carcinoma.
Final Diagnosis:
Metastases, Pulmonary from Testicular Carcinoma
Follow-up and Prognosis:
The patient had an uncomplicated post-operative course. He was
treated with chemotherapy.
