An unenhanced high resolution chest CT exam obtained 17 days after the initial diagnosis shows a loculated large right pleural empyema that contains air pockets, an extensive consolidation of the right lower lobe with multiple air pockets within it giving the appearance of necrotic lung, and one of the air pockets in the right lower lobe appears to be in continuity with the pleural space suggesting a bronchopleural fistula.
